Throughout the month of May, Lake Regional Health System will conduct a survey to assess community health needs. This survey is intended to determine the extent of chronic disease, such as diabetes and heart disease, within our community and the underlying causes, like obesity or limited access to care.

“We need input from all segments of our service area to ensure the Community Health Needs Assessment paints an accurate picture of our collective health status,” said Michael E. Henze, CEO of Lake Regional Health System.
“This information will help us improve Lake Regional’s existing disease management programs, like diabetes education. It also will help us identify gaps in our services.”

Under the Patient Protection and Affordable Care Act signed into law in March 2010, nonprofit hospitals nationwide must conduct a comprehensive Community Health Needs Assessment beginning this year, with follow-up surveys conducted every three years.

The survey is available electronically. To participate, visit To complete a paper survey, visit your nearest Lake Regional clinic or pharmacy, or call 573-348-8385 to request a copy be mailed to you.
